Breast Cancer: Understanding Causes, Prevention, and Early Detection

Causes

  • Hereditary- about 5-10% of breast cancers are hereditary, due to mutations in genes such as BRCA1 and BRCA2. 
  • Prolonged exposure to estrogen elevates breast cancer risk- eg. Early menarche (before age 12), late menopause (after 55), nulliparity, and delayed first childbirth. 
  • Postmenopausal hormone replacement therapy, particularly combined estrogen-progesterone regimens  
  • Postmenopausal obesity (elevates risk by 20–40%) 
  • Alcohol intake and smoking 
  • Chest radiation before age 30 (e.g., for Hodgkin’s lymphoma)
  • Endocrine-disrupting chemicals (Eg. bisphenol A (BPA) and phthalates- increases the risk

Prevention

  • Maintaining an ideal body mass index (BMI) 
  • Avoid alcohol and smoking
  • Engage in regular exercise (at least 150minuts/week)
  • Tamoxifen reduces breast cancer risk in high-risk women
  • Prophylactic bilateral mastectomy reduces breast cancer risk in BRCA carriers

Symptoms of Breast Cancer

  • Painless lumps in the breast or armpit
  • Inverted nipples
  • Bloody discharge from the nipple
  • Unusual changes in breast shape or size or changes in the skin
  • Swelling of the upper limbs

How to Screen for Breast Cancer?

Breast cancer screening includes:

  1. Breast Self-Examination (BSE): Regularly checking for lumps or abnormalities at home.
  2. Physician-Assisted Examination: Conducted during routine medical check-ups.
  3. Mammograms: The most effective method for early breast cancer detection.

If abnormalities are detected, an oncologist may recommend further testing or a biopsy.

Screening Guidelines by Risk Level

For Average Risk Individuals (Lifetime Risk <15%)

  • Ages 50-70: Mammogram every 1-2 years.
  • Ages 40-50: Screening may be considered depending on individual health and risk factors.
  • Ages 70+: Mammograms recommended only for women with a life expectancy of at least 10 years.
  • Below 40: Routine screening is generally not advised unless risk factors are present.

For High-Risk Individuals (Lifetime Risk >20%)

Includes:

  • BRCA1 or BRCA2 gene mutation carriers.
  • First-degree relatives of BRCA gene mutation carriers.
  • Individuals who underwent chest radiation therapy between ages 10 and 30.
  • Those with hereditary conditions like Li-Fraumeni Syndrome.

Recommendations:

  • Begin screening at age 25.
  • Annual MRI mammograms are advised in addition to physical exams.
